ANNOTATIONS

The 2024 amendment, approved by Supreme Court Order No. S-1-RCR-2024-00063, effective for all cases pending or filed on or after October 7, 2024, provided sections for individuals to indicate their preferred salutations and preferred pronouns, and made certain stylistic changes; in the first undesignated line, substituted "Badge" with "ID"; in Question 1, after the question number, added "Salutation (optional - Ms./Mrs., Mr., or Mx.)"; and in Question 2, before "gender", added "Pronouns (optional - he/him/his, she/her/hers, or they/them/theirs) and". The 2019 amendment, approved by Supreme Court Order No. 19-8300-022, effective December 31, 2019, rewrote the form to the extent that a detailed comparison is impracticable.
